SCHEDULING

New patients are those I have not seen before or have not worked with in the last 3 years.

Established patients are those I have worked with within the last 36 months.

I understand that life can be complicated and that unexpected events can happen.  If you are unable to make it to your in-person or telehealth appointment, I request 48 hours' notice. Your appointment time is reserved just for you and a late cancellation or no-show prevents another person from using that time slot.  A credit card is kept on file and you may be charged a $50 fee for a no-show or late cancellation.

Your first visit is an opportunity to start building a new relationship with your health. We will:

• Discuss your health goals and expectations. What brought you here and what do you hope to achieve?
• Perform a thorough review of your health history, including your current concerns, your past medical history and family medical history.
• Review your current lifestyle and dietary habits and consider how they may contribute to your overall health.
• Review any recent lab results from other providers.
• Order any necessary lab work or functional assessments if we need to gain more insight into your health.
• Outline next steps.
