Accepting Multiple Insurance Plans
I am in-network with multiple insurance companies, but your benefit coverage and financial responsibility will depend on your specific insurance plan, deductible, copay, or co-insurance. A provider’s preferred status with an insurance company does not always guarantee coverage. Patients are responsible for verifying their benefits and coverage with their insurance company prior to their initial visit.
I am in network with the following insurance companies: Aetna, Coordinated Care Ambetter and Cascade Care, First Choice (coming soon), Lifewise, Multiplan, PHCS, Premera, Regence UMP, Regence WA, Uniform Medical, United Healthcare.
Copays are due at the time of service. I accept cash, check, credit or debit.
I offer a 20% discount on office visits not billed to insurance, when paid in full on the date of service.
The length of your visit includes all time-related activities on the date of your visit. This may be beyond our face-to-face time.
Supplements and some laboratory tests are not covered by insurance companies and are payable upon receipt. You may use your health savings plan for supplement purchases.
Ranges from $200 - $240 (after 20% discount, $250 - $300 if payment is not made at the time of the visit), depending on time and complexity. Included in the first visit is a 45 - 60 minute consultation, analysis of recent past lab work and medical records, coordination of care, and a comprehensive treatment plan.
Range (after the 20% discount) from $96 for 20 - 29 minutes of consultation and coordination of care, to $140 for 30 - 39 minutes of consultation and coordination of care, to $180 for visits requiring 40 - 54 minutes of my time.
Telehealth visits are for established patients who are in the state of Washington. Telehealth visits are billed like in-person return office visits.
